Using this plugin you have to add shortcode in your page now click on take picture and click on upload button, now image will be upload in wp media web_cam_media_id using this hook you can get uploaded image media id so you can customize your self like upload avtar image set product image get product image feedback many more using this plugin

for multiple form or page use pass unique slug in shortcode so when you recieve data web_cam_media_id in this action hook you can get in which page of form this media comes and put if else condition according to your requirement

web_cam_media_id use this action to get capture image id on function.php file
